I have found this only on blogs where lots of plugins are active. I'm not exactly sure what interaction is causing this. Thankfully the fix is to disable and re-enable the plugin. <br>Another plugin called WP Super Edit also provides many of the same features but I have seen the same issue there. <br><br>All in all - I'd recommend giving it a shot.
